Advanced directive is the general term that refers to the various documents that include a living will, instruction directives, health care proxy or health care power of attorney.
We are providing links to download Advanced Health Directive forms for your use. The Advanced Health Directive forms are provided by Caring Info a National Hospice and Palliative Care Organization which is endorsed by AARP. Since Advanced Health Directives are not portable between states, there are links for each state.
Some states combine the two forms so you can record your treatment preferences and name your health care advocate in one document. When you download and/or print your state’s form you’ll find what you need.
